Patient Banner

The Patient Banner always displays when a patient is selected and is the same standard patient banner used when accessing Vision 3 Apps from Consultation Manager. It displays the following patient demographic information:

• Name - Surname and forename 1 (title)
• Born - Date of birth (age)
• Gender
• NHS/CHI/H+C No
• Address
• Phone – Displays Home telephone number if recorded
• Language Spoken - Displays next to Phone if recorded

Click anywhere in the banner to expand and show more detail. The expanded banner shows the above details plus:

• Address - In full
• All phone numbers - If recorded
• Email - If recorded
